Designed to give members and their dentist a better understanding of the benefits payable under the Dental Plan before services are provided. A pre-treatment review is recommended if dental services are expected to cost $500 or more, or for certain treatments including bone surgery, bridges, crowns, inlays (post and core) and onlays, periodontic procedures, and veneers. For any of these treatments, we recommend that the member’s dentist provide a proposed course of treatment and a pre-treatment estimate.
