Starting Jan. 1, 2022, your vision benefits plan will switch to SuperiorVision.
Search the SuperiorVision network for a provider. When searching for a SuperiorVision provider, please note that the network to search if prompted is the “Superior National” network and the coverage type if prompted is “Insurance through your employer”.

Find a Pharmacy
Search for a network pharmacy near you. Registration is required for first time use.

Take your Health Risk Assessment (HRA).
You will need to use the default password: hrasuburban to get into the system. Follow the prompts to reset your login and password. If this is your first time completing an HRA, you can set up an account by clicking I don’t have an account: Sign-Up.
